In an effort to expedite the registration process, you may download new patient forms from this website. Just print, complete, and bring the forms to your first visit.

New and Returning Patients

As your medical care provider, we are interested in ways we can best serve you. Please take time to read the instructions below as they will affect how we can care for you.

• Please bring your current insurance card with you to each visit. Updates are required at each visit.
  
  
• If your insurance requires a referral, please arrange for this with your primary care physician prior to your appointment time.
  
  
• If your insurance requires that you pay a Co-Pay, you will be expected to pay at your appointment time.
  
  
• Bring a list of your current medications and current eye glasses.
  
  
• If you have a Vision Plan for routine eye care, please call our office before your visit.
  
  
• Contact Lenses and Fitting Fees may be incurred during your visit and are not generally covered by insurance. You will be required to pay for these at your appointment time.
  
  
• Wear your Contact Lenses to your appointment. Bring your current eye care solutions with you.
  
  
• If the patient is a minor (under the age of 18) they must be accompanied by a parent or guardian.
